3 of my photos on the last sortie were overexposed. What determines whether they’re overexposed or not?

I am guessing here, but maybe height you are flying at is a contributor. It could also be just a random calculation that OBD has applied.

There's a random calc going on, all that vibration, stress etc, bullets can cause some errors! Possible the plate cover is damaged and so on.

last i checked, did not those cameras have a fixed shutter and aperture?

I don't think you can do anything about it in the game, it is just a chance occurrence.
