I got this error while firing cannon in Comanche.
Does it happen every time?

Yes. I repeatly get these two errors in the exactly same situation : Firing 20mm cannons at multiple vehicles and infantries at Enemy FARPs as comanche. but it doesnt happen always, just sometimes. Do you have any clue what these errors mean ?
I didnt install any mods other than 1.16.1 patch and 1.16.2 patch.
Which map is it on?
Does it happen on other maps?
Does it happen when you fire the cannon, or when the shots hit the target?

I was playing Alexander archipelago FJORD WAR campaign. the error both happened while firing or after firing so it might be happening when the target is hit.
I'll see is it happening in other maps as well, but if this error is not related to maps it might happen in same way.
I didnt installed new voice MODs before, but when I included it when reinstalling the game this issue didnt happen anymore when I repeatly playing FJORD WAR campaign.
I think the modders modified the game to be worked with their new voice mods and it caused the game to be broken with default voices.
Sorry, but you won't get any further help from Messyhead as he has stopped working on the game and has actually left the forum due to the attitudes and comments of some of the members. In particular Banita who, in my opinion, was really offensive to him.
All I can think of is - do you have OpenAL installed ? I know that is required for the new sounds. I think it is automatically installed with the 1.16.0 upgrade.
#4 When I first installed the game my X52 Pro worked perfectly after running through the basic contoller set up in EECH. For some reason out of the blue, now the Z-rotation on the stick mimicing/replaceing the rudder now pegs all the way left. In Windows all flight stick/HOTAS calibration/test settings work perfectly. I've had to disable the controllers using rudder entirely and am using the keyboard for rudder control.
You can't select the axis you want, say, like the X52's stick twist, in the Controls screen? You previously had to mess with the ini for that, but now you should be able to assign in-sim. You can even test it on-screen.

You realize you need to install 1.16.0 first after the stock version, right? I skipped that step after reinstalling recently and had a bunch of errors until I re-installed properly. The only consistent CTD/freeze I get now is Comanche + ASE page + HMD padlocking + rolling, which incidentally is a pretty common set of conditions when flying a CAP in a Comanche, but if I just flip away from the ASE when dog-fighting in the Comanche or don't use the HMD at all in that same bird then it won't happen and 1.16.2 is fairly stable now.

The term "necroposting" was invented by a person with no social memory beyond a year. People with a similar hangup are those o.k. with the internet being transient vapor.
I found that some trackIR axes are not working in each helicopters.
Only comanche has all moveable axes,
Apache has no roll, Viper and other helicopters have no x,y,z.
I've just started using TrackIR, having finally bought a IR clip and converted my PS3 camera. It's works really well.
I'll check out the axes in those helos, I tried a few and thought it was ok. One thing I did find is the pitch up and down seems to be limited, so I'll see if I can fix that.
Finally get a consistent crash again every time trying to enter any Cuba campaigns using DEBUG1162.exe. Disabling the current game.cfg, EECH.ini, GWUT, and playersv.bin with appending bck to their names had no effect on this. I was briefly able to get into these campaigns with the COHOKUM1162.exe but then it froze and wouldn't let me back in.

Can you just attach the debug log instead of copying/pasting?

This is only happening in the Debug exe (I tried it too). Does it happen with the cohokum1162.exe for you? I tried it with cohokum1162 and it didn't crash.
If it's only the debug exe, then I don't think it's too big a deal, as that is only for testing.

Eventually it crashes in Cuba in the non-debug version, which is why I was trying to run the debug version to find out why.

Is it the same error in the main exe? You can enabled debug_log in the ini file
Looks like it.
Edit:
Yep. Just got another freeze/CTD with the non-debug version. Definitely the same thing. Happened on a recon mission right before getting to the X way point with enemy choppers in the distance. Flying a Comanche in Cuba. This time, this is all that's inside the debug_log.txt:

Ok, thanks. I'm not sure how the terrain code all works, so I'll see if I can figure out what's going.
Also, just fyi, the file path is a red herring as that just comes from the project map that's made when the code is compiled, and uses local paths when it's compiling. This is the relevant bit \modules\3d\terrain\terrmap.c:1692

Hah hah, even our debug loggers have bugs.

Right now, though, it keeps freezing at the end of a P.R. campaign mission, also in a Comanche, but after sinking a ship in a very long stealthy mission as I was coming into land just as I pass the middle of the airfield. No debug error logs for this one, though. Happens at exactly the same spot when I bank just slightly over the field. I got no freezes for hours, though, flying as just a Hokum A and B on the red side.
Edit: If I just stay at the outside of the airfield and I land like just outside the taxiways I'm fine and it doesn't freeze. The biggest issue seems to be flying over the middle of the runways. Also if I back out of the helicopter and I in the mission, it will just fly around a circuit above the helicopter area and then land and it's still fine.
