A friend and I were doing a coop B-25 escort mission a couple of days ago and when the B-25's were on their bomb run I saw something happen, luckily I was recording at the time and here is the playback. Have you ever seen this happen in game?

We used to use that as a tactic online but I've never seen AI drop bombs on other planes accidentally lol. My guess is one had engine damage and fell out of formation just at the wrong time?

They had just earlier been hit by enemy fighters, I had just shot one down and was a couple of thousand yards behind the bombers closing on them when BOOM! I think my jaw literally dropped.

Ok here is another oddity from about a month ago. My flying buddy is in the lead and I'm a few hundred yards behind in my Fw-190 returning to base after a fighter patrol, we decided to beat up the countryside on our way back and just happen to overfly a friendly 88mm flak site.....

Had another oddity happen last night. We've made numerous attacks on bombers of all types in IL2 and NEVER seen anything like this happen.
I perform a head attack vs against the lead A-20 in this formation getting good hits on the starboard engine, and then:

Taking out the lead plane can cause all sorts of chaos, as you found out.

I've done that, shot down the lead, many times before and never have had this reaction but this one time.
