Definitely something screwy with Windows 10 and NVIDIA. My issue is probably slightly different because of I am running surround mode and also not running an upgraded version from Windows 8 to 10.

Ok I've performed extensive testing in surround mode with NVIDIA Windows 10 drivers from 355.80 to 361.43 via the MSI GTX980ti. I have found that this problem exists in all iterations of patches in this range for the 980ti three monitor configuration under Windows 10. I also figured out what's causing the inability to disable surround mode and eventually causes the NVIDIA control panel(NCP) to crash. What's happening is if you have three monitors and disable the surround mode, the NCP gets confused and fails to disconnect from all three screens. The way to effectively prevent the surround mode from getting stuck is to first shutoff all displays except the primary one. You do not have to unplug them from the NVIDIA card. The primary display is the one that you chose as your main display. It is denoted by an asterisks in the NCP. Then you have to click restore defaults in the upper right. Do not click the button labeled disable surround mode. It is buggy. Then you can uncheck the tick box for surround Physx . Click "Apply" changes, and exit out of the menu. Go back into NCP and you'll notice that the tick box will still be checked. Do this one more time and then your screen will be set to non-surround mode. I believe that these drivers were well tested in one and maybe two monitor configurations since these are most likely the largest number of setups in use. Triple monitor users are fewer in number in terms of the market and as such the disable enable button was not effectively tested on three monitor setups. Right now, this probably is an under reported bug since not many users have both windows 10 and triple screen configurations.

Hey Ice, I updated to 4.33 U5 and it refuses to use my surround settings. It is displaying on one monitor only. I have 5760x1080 set both in game and in my NVIDIA surround settings. The older 4.33 is still working correctly(separate install) and is displaying across all three monitors. I've played around with the g_fMaximumFOV and set it to 140 just like the 4.33version. Still, not working...
