Yep even had one in training, thx god, hit the door frame, then land at our feet.
Also had a window smashed, thrown in and landed in a babies crib, again, Thx god no baby was there. That would of been ugly.

I live in a nice neighborhood but there is a "sketchy" family that lives about 4 houses away and one morning, around 6am, my wife and I woke to what sounded like a couple of bombs. I mean LOUD! Turns out they were flashbangs thrown into the sketchy family's bedroom windows by the local SWAT team. Turns out the neighbors daughter and boyfriend were dealing a "controlled substance".
Anyway, I'll tell you this, the movies and video games do NOT do the sound/volume of a flashbang justice. They are extremely loud. Shockingly so. If it was enough to wake both me and my wife from several hundred feet away, I can only imagine what it must have sounded like inside that house.

I probably throw or in the presence of flashbangs 8-12 times a month on our team. You get used to them, you don't even think about its after awhile. Lots of funny stories about what the bad guys have done. Admittedly, although we haven't had a serious accident or injury yet, we kind of use a slightly increased discretion after the unfortunate incident in Atlanta this summer, with the baby in the car seat. Four in one house, I never seen the need for. Two is uncommon. By the time you threw the 4th one, the effect is reduced, and the team should be dominating at least the lower by then.
