It's possible to be killed bailing out, I daresay that there is some random number generator that calculates your chances of surviving a bailout that was at play here. Also, 1,000ft is too low to be "safely" bailing out.

I understand about the files, but is it too late to do it as I was surprised that it allowed me to fly another mission, which I did. Let me know please.
To better clarify: I understand that it is possible to die while bailing out, and also after injury. After the mission where I did bail out, I looked at the mission details and it stated that I had "Bailed Out" and that my status was "Killed". Now normally after that, as I know you are well aware, if you try to go to next mission it tells you that you're dead and to enlist a new pilot. This did not happen. I flew 1 more mission, where I chalked 2 more kills (which btw it registered in the pilot log as pending), landed safely (at home field which is what it stated in details), and then it did the "you're dead, enlist new pilot etc".
I have the outcomes setting always at normal, not Pilot never dies.

I will be watching for this also. I recently had a very similar case with a German Pilot.
Insanity: doing the same thing over and over again and expecting different results. A. I had this happen again. I sent an email with logs and screen shots to OBD. Unfortunately I did not get all the logs. Will do better next time.
This was a German Pilot flying over England. Got hit by blast damage, wounded to 54% I think. Continued flying until ran out of fuel due to damage. Bailed out over England.
State in the duty room image shows Bailed out and Killed. But at the bottom all that is said is the pilot was pretty shaken up. Nothing about being Captured. Nothing about Spending time in the Hospital. Exiting the duty room screen I was able to advance to the next mission and fly it. I did not try to fly a second mission after being dead. Viewing the pilot show that he is dead there. Stopping and starting WOTR resulted in the pilot being really dead.
