When you have this many incidents, there is something wrong. Yes, they have a lot of students, but still the rate is still too high. But then, student pilots always have a higher rate. Leaving the ground has some opportunities for unhappy outcomes. I have seen the Dean aircraft and they are ragged-out. Old Army adage - “if it looks bad, it is bad. If it looks good, it might be good.”

Seems like it may be a low wing and high wing conflict? Add low-time and not enough watching outside...

All reports appear to offer same opinion of the outfit as the OP.
