Commanding Officer, Executive Officer and Master Chief Petty Officer relieved of command and more will likely face charges " The US navy relieved the USS Fitzgerald’s captain of his command and other sailors were punished after the navy found poor seamanship and flaws in keeping watch contributed to the June collision"http://www.reuters.com/article/us-usa-navy-asia-idUSKCN1AX2TS

rules of the road are not all that complicated and include not hitting another ship or being hit by one. I'm sure they have more advanced plotting than what we used in the 70's, some major failure there.

All those who were supposed to be standing watch were probably in diversity training.

President Trump, asked about the incident as he returned to the White House from a two-week working vacation on Sunday, said, "That's too bad." Yeah, I'm sure that's all he said.

"The USS John S. McCain's sister ship, the USS Fitzgerald, almost sank off the coast of Japan after it was struck by a Philippine container ship on June 17"
when it rains it pours ? this time it was hit by an oil tanker
this could be the worst year for the U.S. Navy in a good while

Are these collisions more frequent than collisions that occurred during the 1990's or 1980's or is it just a case that all of these incidents get reported on now due to our 24/7 news cycle and internet world? Think it's more to lack of proper training. Several captains have been removed in recent years from their posts after lack of discipline ranging from pushing the crew past their limits or leaving their posts when they should be on duty.

I served on a U.S. Navy destroyer and at times stood watch as "port lookout", "starboard lookout" and "helmsman". We once had a Junior Officer who was on the bridge as "OOD" = Officer of the Deck - either he or the Quartermaster goofed and got us too close to the shallows and the bottom of the ship hit a sand bar in the Caribbean.
they are not sure yet what caused this latest collision but standing a proper watch (that includes having binoculars) can be a critical factor. It is believed that the guy in the crows nest on the Titanic had no binoculars - if he had them he might have seen the iceberg in time

Thanks for that info. Now the question to ask is this lack of proper training due to budgetary reasons or due to changes in the overall culture of the Navy?

Did these incidents happen at night? I don't think so because if so, we would be talking NVGs and not binoculars. So assuming these things happened during the day, how long does it take really for an oil tanker or any big ship to get from the horizon to right-in-your-face? Nobody noticed them during that length of time? Nobody made any evasive maneuvers? I'm thinking even if these things don't turn on a dime, won't being hard-to-port or hard-to-starboard for the past 5-10 minutes be enough to change course enough not to scratch the paint?
Sorry if I'm off here, no clue at all about the navy or big merchant ships, but just coming at this with some common sense.

My nephew had read that the ships radar had failed earlier that day, you would think they would have extra people on watch if that's the case. He mentioned when he was being deployed to Kuwait as a Marine the ship he was riding on didn't have their Surface to AIR radars on while an Iranian P-3 buzzed them. Another time a guy manning a 50 cal was asleep while 6 blacked out Iranian fast boats were charging them. It sounds like the Navy is getting way to laid back... I had read the accident happened at 6:24am, I would think that is bright enough to see that large of a ship coming at you...
Plus, why didn't the merchant ship move and avoid the destroyer? Almost seems like these ships are trying to weaken our naval presence over there.
