What mode is the external screen operating on? Is it expand view or mirror (same picture on both screens) ?
Do you have V-Sync on in the graphics card options?

Right click desktop -> Nvidia Control panel
Manage 3D settings -> Global settings
Vertical Sync = Set to On
Does this make a difference at all ?

There is the problem your driver install is borked if you cannot get the control panel up download latest nvidia drivers then get this http://www.guru3d.com/files-details/display-driver-uninstaller-download.htmlreboot PC in safe mode (use f8) if using windows 7 run driver uninstaller and after the reboot , try new drivers , do custom install tick the clean install option see how you go
