I resigned from a job 3 months ago, after 6 years there.
My letter was:
Please accept this letter as notice of my resignation, effective 16 Jun, 2014. I am willing to stay on the customary two weeks to close out my current projects.

I have no idea what your situation is, what kind of job you are doing, where you are going, how business associations relate, etc; however it is never good to burn bridges. Even if it would be presently gratifying, it almost never works to your benefit. Short, simple, respectful, and appropriate.
Good people sleep peaceably in their beds at night only because rough men stand ready to do violence on their behalf.
Someday your life will flash in front of your eyes. Make sure it is worth watching.
