I'm having difficulty getting my GBU to release. Generally, I will have everything set up perfectly on first pass, and bomb doesn't release (fall line designator comes down, flashes, pops back up, falls down but goes up again without releasing). Strangely, if I keep the target locked, fly over target, then turn around, it almost always drops on the second pass. Happens EVERY TIME. Any other constraints I need to keep in mind? Usually dropping from 15,000 feet...

Are you guys holding down the wpn release? There is a bug that if you keep holding the pickle for too long, it won't command weapons drop. You two should try pressing down about 2 seconds before the release cue touches the velocity vector!

So should I hold down the pickle while the cue drops the 1st time.. then release it and then hold it down again while it drops the 2nd time?

No it works for the first time as well. If you're using BMS you also have a few seconds after the release cue reaches the FPM to drop, so if they don't drop with the first try, immediately release and repress the pickle button and it should work.
