I finally got the build environment working and I have confirmed that the problem is with the IDirectDrawSurface7_Blt call in zbuffer.c. Thanks to Lucas and Arneh for the help. Big thanks to Chamot for the clue!

I did a very simple test.

in zbuffer.c, there are two calls: clear_zbuffer_screen and clear_zbuffer_screen_using_viewport. clear_zbuffer_screen uses IDirectDrawSurface7_Blt. I simply made a copy of clear_zbuffer_screen_using_viewport and renamed it clear_zbuffer_screen and when I now run the demo, I actually see something!

Given I'm not really a DirectX programmer, I would like to leave the real fix to the dev team. I'm not sure what the implications are of the fix or how to really regression test this. I think also, there is probably a cleaner way to fix this but I was also having trouble finding documentation so I could implement IDirect3DViewPort::Clear2. I'm willing to keep fooling with it but I would rather leave it to the professionals!

I have some family stuff this morning, I will get back at in a couple of hours.

Works like a charm on my G965 (Intel GMA X3000).
Thanks for this fix, been waiting for over a year for this.

The fix is included in 1.11.1 so just use that (or later versions from now on).
